There is an increased risk for
malignant brain tumors and for use of mobile phones
Dear Colleagues and readers,
Almost everyone has a mobile
(cellular) telephone nowadays and there may be more than one phone per
person in many countries. The proliferation of use has not been
tempered with health concerns, rather on the technical development in
this area. Since the brain is the organ with the highest near-field
exposure to microwaves during the use of both mobile and desktop
cordless phones an increased risk for brain tumors has been of concern.
Several studies and especially studies from Sweden by Hardell L et al.
have shown to clearly indicate an increased risk for both
malignant brain tumors and acoustic neuroma for long-term use of
wireless phones2. The risk was most pronounced for
ipsilateral tumors after a latency period of >10 years. These
case-control studies have been published previously2,3,5.
In
contrast to the findings for mobile phones no statistically
significantly increased risk was found for cordless phones, although an
increased risk was found in the highest exposure group2.
According to the
studies done by Cardis et al. the average relative SAR* is highest in the temporal lobe
(6–15%, depending on frequency, of the spatial peak SAR in the
most exposed region of the brain) and the cerebellum (2–10%) and
decreases very rapidly with increasing depth, particularly at higher
frequencies. The SAR distribution appears to be fairly similar across
phone models, between older and newer phones and between phones with
different antenna types and positions1.
For the time being,
evidence collected in epidemiological studies points rather to
an underestimation of risk from mobile phone use and less bias
in the Hardell studies as compared to those of the Interphone
group that are affected by selection and possibly also by
misclassification bias4.
* SAR is
the specific energy absorption rate i.e. energy absorption rate per
unit mass (measured in W kg−1).

Hemen hemen herkesin bir cep
telefonu vardır bugünlerde ve hatta bazı ülkelerde tek bir
kişinin birden fazla telefona sahip olması bile olasıdır. Kullanımın bu
derece artışı bu konuda teknik gelişmenin verdiği haz derecesinde
sağlık sorunları ile hafifletilmemiştir.Cep ve kordonsuz masa
telefonlarının kullanımı sırasında mikrodalgaların en yakın organ
olarak beyine etki etmeleri artan bir beyin tümörü
oluşturma riskini de taşımaktadır. Özellikle İsveç'ten
Hardell ve ark. tarafından yürütülen birçok
çalışma açık olarak hem anaplastik beyin
tümörü hem de iyi huylu olan akustik nörinom oluşma
riskini uzun-süreli cep telefonu kullanımında artışını ortaya
koymaktadır2. Bu risk aynı taraftaki tümörler ve
10 yılı aşan bir kullanımda en çok ortaya çıkma
eğilimindedir. Bu olgu-kontrol çalışmaları daha önce
yayımlanan çalışmalardır2,3,5.
Mobil
olan cep telefonlarının aksine kordonsuz masa telefonlarında
istatistiksel olarak bir risk ortaya konamamış olmasına karşın
yüksek oranda kullanım olan gruplarda artan oranda risk bulunmuştur2.
Cardis ve ark.
tarafından yapılan çalışmalar SAR* diye kısaltılan değerin
yüksek frekanlarda en yüksek olarak temporal lob
üzerinde (freklansa bağlı olarak yüzde 6 ila 15
arasında ) ve beyincikte (%2-10) saptandığını ve derine
inildikçe hızlı bir şekilde azaldığını ortaya koymuştur. Telefon modellerine, eski ve yeni
olmalarına, değişik anten tip ve yerleşimlerine bağlı olmaksızın SAR
değerlerinin benzer dağılım gösterdiği saptanmıştır1. Şu an için,
epidemiyolojik çalışmalardan toplanan kanıtlar ışığında mobil
telefon kullanım riskinin önyargısından çok seçim ve
olasılıkla kötü sınıflamalı ikilem tarafından etkilenebilecek
Interphone grubu ile kıyaslandığında Hardell
çalışmalarının daha az ikilemli olması gösterilebilir4

Nanotechnology fights cancer –
Functionalised single-walled carbon nanotubes, rather than being a

tek kayıt altına alınıyor. Bu kayıtlarda, önümüzdeki
aydan itibaren doktorlara e-posta olarak gönderilecek.
Mektuplarla, doktorlara, ‘ne yaptığınızı biliyoruz’ mesajı
verilecek.
İLAÇTAN 2.5 MİLYAR LİRALIK TASARRUF
SGK Başkanı Emin Zararsız, Star’a,
sağlık alanında alınan tedbirle ilgili bilgi verdi. Son dönemde,
ilaç kararnamesi ve ilaçtaki karekod uygulamaları ile
yıllık 2.5 milyar liralık bir tasarruf elde ettiklerini kaydeden
Zararsız, tartışma konusu olan karekod uygulamasında 15 Haziran
itibariyle yüzde 78.8 oranında başarıya ulaşıldığını kaydetti.
Zararsız, ilaç giderlerinin aşağıya çekilmesi
için, denetim sistemini daha da güçlendirdiklerini
ifade ederek, bu çerçevede doktorların yazdıkları
ilaçların, hangi şirketten olduğu, hangi eczaneden alındığının
günlük olarak izlenebildiğini kaydetti. Zararsız, bu sistem
sayesinde, ilaç kullanımında ortaya çıkan
usulsüzlüklerin önüne geçileceğini
kaydetti.
DOKTORLARA GÖNDERİLECEK
SGK, ilaç takip sistemiyle elde edeceği
bilgileri, doktorlara bir mektupla gönderecek. Konuyla ilgili
hazırlıklar halen devam ediyor. Mektuplarda, doktorun bir ay
içinde hangi ilaçları yazdığı, ilaçların hangi
firmalara ait olduğu ve hangi ecznalerden alındığına ilişkin bilgiler
yer alacak. Bir anlamda doktorun, ilaç raporu doktora sunulmuş
olacak. SGK, böylece daha kontrollü ilaç yazılmasını
sağlamayı amaçlıyor.
Sistem SGK tarafından geliştirildi
Doktorların yazdığı ilaçların
izlenmesine ilişkin sistem, SGK Başkanlığı tarafından geliştirildi.
Sistemde, tüm doktorların bilgileri girildi. Girilen bilgiler
doğrultusunda her doktorun bir dosyası oluşturuldu. Dosyalarda,
doktorların yazdığı ilaçların da bilgileri toplanıyor.
Örneğin bir doktorun, bir ay içinde yazdığı
ilaçların listesi dosyasında tutuluyor. Bilgisayar, aynı
doktorun ilaçlarını firma ve alınan eczane bazında da ayrıma
tutuyor. Böylece, bir doktorun hangi firmanın ilaçlarını
yazdığı da tespit edilebiliyor. Söz konusu doktor, hep aynı
firmanın ilaçlarını yazıyorsa sistem not ediyor. Aynı sistem
sayesinde, vatandaşların kimlik bilgilerinin kullanılarak, usulsüz
ilaç alımının yapılmasının da önüne geçiliyor.
How Many
Antiepileptic Trials are Warranted Before Considering Surgery?

Among all patients
with epilepsy, between 60% and 70% are expected to become seizure-free
with antiepileptic drugs (AEDs) and often can discontinue use of
medication. The positive therapeutic expectations of AEDs vary
according to the type and age of onset of the epileptic syndrome. For
example, 80% to 90% of patients with idiopathic generalized seizures
are expected to become seizure-free, whereas this is the case in only
50% of patients with seizure disorders of focal onset. In this latter
group, seizure-freedom may decrease to 30% in cases of temporal lobe
epilepsy secondary to mesial temporal sclerosis or be as high as 95% in
cases of benign rolandic epilepsy of childhood.[1]
Epilepsy surgery has
transformed the therapeutic expectations of partial seizure disorders
that fail to remit with AEDs, particularly when seizures originate from
mesial temporal structures; in the latter scenario, 60% to 70% of
patients can expect to be free of disabling seizures (complex partial
and secondarily generalized tonic-clonic seizures).[2]Early identification of
patients with partial seizure disorders that are resistant to AED
therapy is pivotal. A landmark study published in 2000 showed that a
first monotherapy trial can achieve freedom from seizures in 47% of
patients.[3] Among the remaining 53% of
patients, a switch-over to a second AED trial in monotherapy yields
freedom from seizures in only 13% of patients, whereas a third trial
increases the total number of seizure-free patients by only 4%.
After a careful
review of the available evidence, the Commission on Therapeutic
Strategies of the International League Against Epilepsy (ILAE)
recommended to its executive committee that the definition of
drug-resistant epilepsy be changed to "failure of adequate trials of
two tolerated and appropriately chosen and used AED schedules (whether
as monotherapies or in combination) to achieve sustained seizure
freedom." This definition was officially adopted at the ILAE's 2009
meeting in Budapest, Hungary.[4] In most patients, it is expected
that drug resistance can be established over 6 to 12 months.
Unfortunately, most surgical series report that the mean period before
patients are considered for epilepsy surgery is 15 years. Given the
data cited above, this is clearly unacceptable.
Many clinicians
consider epilepsy surgery as the "the treatment of last resort." Yet,
if the likelihood of seizure remission is minimal after 2 unsuccessful
AED trials, it is reasonable to consider of surgical treatment at that
time, because the chances of seizure remission would increase from 4%
to 60% if the patient was found to be a good surgical candidate.
Furthermore, patients and their families must understand that AEDs are
associated with a variety of adverse events (some serious) and that
persistent seizures are associated with a relatively high risk for
sudden unexpected death in epilepsy ranging from 1 in 150 to 200
patient-years.[5]However, complete
seizure remission can lower the risk for death to that of the general
population.
In summary, 2 trials of
AEDs at optimal doses with the appropriate medications are sufficient
to consider referral of patients with a focal seizure disorder for a
presurgical evaluation.
To be able to recognize the
source of a sound — a flute or a train, say — we first have
to learn that sound's acoustic characteristics. Scientists in France
report that such learning is subconscious, rapid and remarkably robust.
Trevor Agus at the
École normale supérieure in Paris and his colleagues
asked 12 volunteers to listen to a series of short snatches of random
noise, and to identify which of the snatches included repetitions.
Their success rate was below 50%.
But when one specific
snatch containing repetition was secretly and randomly interspersed at
frequent intervals, the listeners quickly became near-perfect in
detecting repetition — even when the snatch was compressed in
time or reversed. Moreover, they retained the memory for several weeks.

Doctors
and nurses
volunteer to provide urgent medical care in countries to victims of war
and disaster regardless of race, religion, or politics.
The
photographs in this slideshow were taken by Michael Goldfarb, an MSF
communications officer, during a two week visit to Haiti in March,
2010. He accompanied MSF field staff on assessments of living
conditions in makeshift camps where people had been living without
basic assistance.
Hundreds
of wounded people arrived in Bishkek hospitals on April 7, following
violent confrontations between armed forces and protesters in the
streets of Kyrgyzstan's capital. The MSF team in Kyrgyzstan responded
immediately by providing the National Hospital and the main ambulance
station with emergency medical supplies and drugs bandages and other
sterile material, intravenous injection sets, antibiotics and
painkillers. More material and drugs are to be donated by MSF today to
three health structures in the capital city.

Resident Traveling Fellowship in
Pediatric Neurosurgery
The Joint Pediatric Neurosurgery Section of the American Association
of Neurological Surgeons and Congress of Neurological
Surgeons has established a traveling fellowship for residents in
accredited
North American neurosurgical training programs. The
fellowship is intended to cover the traveling and living expenses
for 1 month for a resident who wishes to gain additional experience
in pediatric neurosurgery during their residency. The
1-month fellowship can be spent at any pediatric neurosurgery
program within North America to pursue an activity which
broadens the resident’s exposure to pediatric neurosurgery,
including
observation at a clinical or research center, participation
in a research project, or any other relevant activity. Two fellowships
per year are awarded on the basis of a competitive evaluation
by a committee of the Joint Pediatric Section. The maximum fellowship
stipend is USD 2,500.
The application must include:
(1) a statement defining the purpose of the proposed fellowship;
(2) written permission from the applicant’s residency program
director
to pursue the 1-month fellowship if it is awarded;
(3) a letter of acceptance from the pediatric neurosurgical program
where the applicant will seek the fellowship;
(4) the applicant’s current CV.
The strict deadline for application submission is October 30,
2010.

field of Spinal disorders, the European Spine

Spine
Journal Award for basic science. Each paper
submitted to the Journal dealing
with basic
science qualifies automatically for this award as soon as it has been
published. The winning paper will be selected at the end of each
academic year (for the next time in 2009/2010) and will be announced in
one of the following issues. The GRAMMER Award will also be presented
at the next EuroSpine meeting. At least one author of the winning paper
must attend at his/her own expense EuroSpine 2010 meeting which is
being held in Vienna (Austria) from September 15–17, to

announces the competition of the biennial Enrico Greppi Award 2010. The
award will be granted to the best unpublished original paper dealing
with epidemiological, genetic, pathophysiological or therapeutic
aspects of headache. The prize amounts to € 10,000 and is open to

Effects of low-field magnetic
stimulation on brain glucose metabolism.

This is a massively important paper as the authors have
asked the most simple question: does the magnetic resonance imaging
(MRI) scanner itself and the gradient switching cause changes in brain
activity? It is amazing that there are hundreds of thousands of papers
using MRI to image and investigate the brain and none has bothered to
ask whether the imaging tool (MRI) changes what is being imaged. It
turns out that it does. This has enormous implications for safety and
for potentially creating a new generation of combined
stimulators/scanners. One should no longer assume that the MRI scanner
does not directly change brain activity while imaging.
I have been interested in combining brain stimulation and
brain imaging for most of my career, and we have done transcranial
magnetic stimulation (TMS), transcranial direct current stimulation
(tDCS) and vagus nerve stimulation (VNS) within the scanner. There have
always been reports of peripheral nerve stimulation with fast switching
gradients, but everyone has just assumed that the MRI scanner does not
actually change brain activity. There is a group at the McLean Hospital
(Renshaw and Rohan[1,2])
who has claimed that some MRI sequences might change mood and treat
depression. This paper cleverly used positron emission tomography (PET)
radiotracer injection within the MRI scanner, with the scanner off or
on and at different gradient switching rates. With PET (and single
photon emission computed tomography [SPECT]), you can inject the
tracer, which distributes roughly according to what the brain is doing
at that moment, and then develop the picture later. Thus, they used PET
to image what was happening in the brain during the MRI scanning and
found that there were focal brain changes associated with the scanner
being on. These changes were proportional to some general rules about
gradient switching strength. This paper has enormous implications for
safety and for potentially creating a new generation of combined
stimulators/scanners. In terms of safety, we can no longer assume that
MRI does not affect the brain. There needs to be a broad reexamination
of which field strengths and gradient switching protocols are safe and
which are not. This study suggests that, if one could better understand
how the MRI causes changes in metabolism, one could create the machines
of the future, where one could scan in one instance and stimulate in
the other, all with the same device.

Abstract
Echo planar imaging (EPI), the gold standard technique for functional
MRI (fMRI), is based on fast magnetic field gradient switching. These
time-varying magnetic fields induce electric (E) fields in the brain
that could influence neuronal activity; but this has not been tested.
Here we assessed the effects of EPI on brain glucose metabolism (marker
of brain function) using PET and 18F 2-fluoro-2-deoxy-D-glucose
((18)FDG). Fifteen healthy subjects were in a 4 T magnet during the
(18)FDG uptake period twice: with (ON) and without (OFF) EPI gradients
pulses along the z-axis (G(z): 23 mT/m; 250 mus rise-time; 920 Hz). The
E-field from these EPI pulses is non-homogeneous, increasing linearly
from the gradient's isocenter (radial and z directions), which allowed
us to assess the correlation between local strength of the E-field and
the regional metabolic differences between ON and OFF sessions.
Metabolic images were normalized to metabolic activity in the plane
positioned at the gradient's isocenter where E=0 for both ON and OFF
conditions. Statistical parametric analyses used to identify regions
that differed between ON versus OFF (p<0.05, corrected) showed that
the relative metabolism was lower in areas at the poles of the brain
(inferior occipital and frontal and superior parietal cortices) for ON
than for OFF, which was also documented with individual region of
interest analysis. Moreover the magnitude of the metabolic decrements
was significantly correlated with the estimated strength of E (r=0.68,
p<0.0001); the stronger the E-field the larger the decreases.
However, we did not detect differences between ON versus OFF conditions
on mood ratings nor on absolute whole brain metabolism. This data
provides preliminary evidence that EPI sequences may affect neuronal
activity and merits further investigation.

MRI set to win reprieve
from EU ban

A controversial piece of European Union (EU) legislation that
outlaws much routine brain and body imaging in research laboratories
and clinics is on course for a revamp.
The EU directive was designed to limit workers' exposure to
electromagnetic fields (EMF) and was approved in 2004. But it was put
on hold in 2007 after scientists and others said that it would stop
them from carrying out important work without improving worker safety.
The NLM Update was held at the Annual Meeting of the Medical
Library Association in Washington, DC on May 25, 2010. There were three
speakers at this year's update. Dr. Donald A.B. Lindberg, Director,
brought users up-to-date on some of the Library programs as well as
future plans; Mr. Sheldon Kotzin, Associate Director for Library
Operations, gave an update on Library Operations activities as well as
selected activities in other NLM Divisions; and Dr. Deborah Zarin,
Director, ClinicalTrials.gov, Lister Hill National Center for
Biomedical Communications, gave an update on ClinicalTrials.gov.
Researchers at Imperial College London found that there was no
association between early childhood cancers and mobile phone base
station exposures during pregnancy.
